I added a small sliver of "Old Olive 1/4" Grosgrain Ribbon" to give my card a little zest of color. I then added my hummingbird by adhering a "Stampin' Dimensional to his underside and placing him on my card.
The next few pictures will show you the "pop-up" inside the card:

I used the "Scallop Trim Border Punch" for one side of my "Whisper White" layer. The popped-up image is the same stamp I used on the front, punch with the "1 3/8" Circle Punch" and create another hummingbird as above, adhere to front of circle.

The sentiment is from the "On Your Birthday" stamp set.

You will need a second layer for the inside in order for this to work. Make this layer a little smaller than your card so it will fit neatly tucked inside. Score in the middle for the fold line.
Using a bone folder, score a rectangle like I did for the picture above. I made mine about 1 inch wide by 3 inches long. With the fold line in the middle, my line was 1.5 inches above the fold line and 1.5 inches below the fold line.

Using an exacto knife or something sharp, cut through your paper the 2 long sides only, leaving the short sides intact. The lines for the short side will be used for folding only.

Picking up your cardstock piece, push your finger through the back to "poke" the piece you just cut. Fold it in the opposite direction than the fold you created for your layer. It should look just like the picture above.

Once all of your score lines have been folded, this is how the card layer will open:
